The length of care depends upon the individual’s medical condition and the type of care ordered by the physician. Services may be provided for two months up to indefinite periods of time. The number of home visits made depends on the medical condition of the patient and the type of services needed. Any person in Franklin, Williamson, Jackson or Johnson Counties who is home-bound due to a medical condition is eligible. Client referrals are accepted from physicians, family, friends, hospitals and social services agencies, provided that a physician certifies that a service is needed.
